The Role of Transitional Care
For older adults, especially those with complex health conditions, transitional care is
critical. During transitional care, health care providers coordinate to help ensure a safe
transition from one health care setting to another, such as the transition from a hospital to a
nursing home. This coordination offers continuity of care and a reduced likelihood of
medication and treatment errors.
Accessing Transitional Care
Your doctor can arrange for Transitional Care. Based on your needs,
your doctor will determine the best Transitional Care setting for
you.
