The Role of Transitional Care
For older adults, especially those with complex health conditions, transitional care is critical. During transitional care, health care providers coordinate to help ensure a safe transition from one health care setting to another, such as the transition from a hospital to a nursing home. This coordination offers continuity of care and a reduced likelihood of medication and treatment errors.
