Nursing Home Care in Your Home
In Pennsylvania, the Program for All-Inclusive Care for the Elderly (PACE), also known as Long-term Care Capitated Assistance Program (LTCCAP), provides an alternative to nursing home care for older adults, age 60 and older, who want to remain in their home and community for as long as possible. This alternative provides care equivalent to the level of services in a nursing home.

About the Program
After enrollment in the program, an interdisciplinary team manages your care. In
some of UPMC service areas, Community
LIFE staff can evaluate your needs and make referrals to needed medical and
supportive services to help avoid costly delays and potential complications. If you
qualify for this program, Community LIFE will initiate the referral process. You or
your physician can contact
us for assistance in making the referrals or you can contact your
county Area Agency on Aging. The services include personal care, pharmacy,
medical equipment,
rehabilitation, and hospital
care, as needed. It is an inclusive care program, based on your
needs.
