What is Considered Long-term Care?
Long-term Care is health care provided over a long period, for the management of chronic conditions. People of any age may need long-term care, and this level of care may involve living at:

  • Your own home (if you have home health support)
  • A continuing care retirement community (independent-living)
  • An assisted-living facility (personal care)
  • A nursing home
Types of Long-term Care Available
There are several types of long-term care available. They include:

Accessing Long-term Care & Retirement Housing
Pennsylvania's Area Agencies on Aging (AAA) assists in finding many of the long-term care resources. The AAA is responsible for assessing the needs of the elderly, determining eligibility for programs and resources, and overseeing care management services. UPMC works closely with AAAs, making referrals on behalf of the elderly as well as providing services. By working with both UPMC and AAA, you may enhance the coordination of care and quality of life.
