What is Care Management?
When we mention care management, we’re referring to the identification and coordination of services and support systems to help older adults with their diverse and changing needs. Care managers can aid in accessing services such as:

  • Healthcare services
  • Mental health services
  • Community assistance
  • Education
  • Vocational training
  • Transportation assistance
  • Legal services
  • Advocacy
  • Respite
  • Recreational services

The goal of care management is to help older adults live more independently, in their own homes, for as long as possible. Care managers are valuable resources because they help older adults avoid premature institutionalization and prevent unnecessary hospitalizations.
